Albury Wodonga Aboriginal Health Service (AWAHS) is a proud Aboriginal Community Controlled Health Organisation, supporting community across North East Victoria and the Southern Riverina. For over 25 years, AWAHS has worked alongside Aboriginal and Torres Strait Islander people to deliver holistic, culturally safe care and create stronger futures.
We are looking for an experienced and motivated NDIS Support Coordinator to join the Social Emotional Wellbeing team. If you enjoy helping people navigate the NDIS, coordinating quality supports and empowering participants to achieve their goals, we would love to hear from you.
About the Role
This is more than coordination, it is about walking alongside people and helping them navigate systems with confidence.
You will support participants with complex needs to access services, understand their NDIS plans, and build the skills to manage their supports over time. Your work will help remove barriers, strengthen outcomes and create pathways to independence.
How you will make a difference:
Coordinating personalised supports for participants and families
Supporting access to the NDIS, including eligibility and plan implementation
Building strong relationships with participants, community and service providers
Advocating for participants and helping them achieve their goals
Working collaboratively across health, disability and community services
Identifying service gaps and helping improve access pathways for community
You are someone who leads with respect, builds trust easily, and is committed to positive outcomes for community.
You will bring:
Experience in NDIS support coordination or working with people with disability
Strong ability to support individuals with complex needs and their families
Knowledge of NDIS guidelines, legislation and community services
A strengths-based, culturally responsive approach
Confidence working with a wide range of stakeholders
Passion for empowering Aboriginal and Torres Strait Islander people
Current driver’s licence (and relevant clearances)
