This position is for AWAHS's, Albury Wodonga’s leading Aboriginal health service supporting the local Aboriginal and Torres Strait community for over 25 years. They are responsive to the community’s varied health and social needs. They are focused on providing the community with a culturally appropriate, ethical & responsive service.
You will:
Coordinate responsive, individualised support to participants with complex needs and their families
Establish and maintain effective relationships with participants and their families, carers and advocates, NDIA, other mainstream and disability service providers, members of the community, networking groups, funding bodies, government agencies, auditors and practitioners
Provide quality support to enable participants and their families to promote the participant’s wellbeing and development and the family’s capacity to be self-managing
We are looking for an Individual who have:
Experience providing NDIS Support Coordination Services in a similar role
A solid understanding of the NDIS and especially its importance to Aboriginal people with a disability and their support networks
Qualifications in the field of health, social or disability are an advantage, but not essential
A passion for positively changing participant’s lives by liaising with the participant, family or representative on supporting the participant with achieving the goals of the Action Plan
Excellent written and verbal communication skills
Strong focus on quality outcomes and building relationships
Demonstrated knowledge of current health issues relevant to Aboriginal & Torres Strait Islander communities
Current Drivers Licence, NDIS Screening Check, WWC and clear Police Checks
